This self-assessment does not count as a diagnosis; it can help you to find out if you have any traits of ADHD. This will also give you a better understanding of how your brain works and for which tasks you may need additional support.

The assessment is adapted from the Adult ADHD Self-Report Scale (ASRS-v1.1) Symptom Checklist that was developed in conjunction with the World Health Organization (WHO), and the Workgroup on Adult ADHD.

The questions are consistent with DSM-IV criteria and address the manifestations of ADHD symptoms in adults. Six of the questions that were found to be the most predictive of symptoms consistent with ADHD form Part A. Part B contains the remaining twelve questions, that describe common ADHD symptoms.

* 1. I have trouble wrapping up the final details of a project, once the challenging parts have been done.

* 2. I have difficulty getting things in order when I have to do a task that requires organization.

* 3. I have problems remembering appointments or obligations.

* 4. When I have a task that requires a lot of thought, I avoid or delay getting started.

* 5. I fidget or squirm with my hands or feet when I have to sit down for a long time.

* 6. I feel overly active and compelled to do things, like I’m being driven by a motor.

* 7. I make careless mistakes when I have to work on a boring or difficult project.

* 8. I have difficulty keeping my attention when I’m doing boring or repetitive work.

* 9. I have difficulty concentrating on what people say to me, even when they are speaking to me.

* 10. I misplace or have difficulty finding things at home or at work.

* 11. I’m distracted by activity or noise around me.

* 12. I leave my seat in meetings or other situations where I’m expected to remain seated.

* 14. I have difficulty unwinding and relaxing when I have time to myself.

* 15. I find myself talking too much when I’m in social situations.

* 16. When I’m in a conversation, I find myself finishing sentences of people I’m talking to, before they can finish them themselves.

* 17. I have difficulty waiting for my turn in situations when turn taking is required.

* 18. I interrupt others when they are busy.
